MEDICAL MASSAGE REQUIREMENTS
In order to bill insurance you must provide a prescription from a Physican with a date, diagnosis code, and number of treatments authorized.  For auto accidents and L&I claims, please provide your claim number, claims manager contact information, and attorney contact information.
 
CANCELLATION POLICY
Email and text reminders (must opt in) will be sent 24 hours prior to your appointment, therefore kindly reschedule your appointment at that time to avoid a $35 fee to your account.
